This snippet is suggesting that AP wanted to switch channel\bandwidth to dual channel, but your wifi adapter couldn't comply with the change, hence forced disconnect.
I suggest you to reconfigure your AP in a way it supports single channel operation only (set it to either 20Mhz or 40Mhz, not both) and also if possible, disable auto-channel function of your AP and manually select operating channel\bandwidth, so your wifi adapter won't have to switch between ever changing channels.
