konsole is missing a Debian menu entry since Sat, 09 Jan 2010.
The file below (/usr/share/menu/konsole) should work:

Hi,

after the CTTE bug #741573 [1], the Debian menu is deprecated in favour
of XDG one.  Since KDE applications ship .desktop files for it already,
and they are doing so for long time, then Debian menu entries for them
will not be added anymore.
