* What led up to the situation?
The drives of a kvm host are mirrored to a partner host with drbd, and ocfs2 is the file system. A kvm host drive was being synced to a partner host with drbd at the time. A single Debian 8 vm exists on the kvm host with two thin-provisioned qcow2 disks. An lxc container was being created for the first time on the vm. debootstrap was installing files on the vm for the container when the failure occurred. This happened a second time after a reboot. It seems repeatable in the current circumstances.
